VG+ - Booklet has wear or meaning a tab indent. Digipak has some wear.

MINT - New Disc has no scratches.
NM and EX - Disc has no scratches barely played. EX may have a light fingerprint or mark that can be removed
VG+ - Disc has some small minor scratches but not affecting play.
VG - Disc has a bit more scratches than VG+ but also not affecting play.
