Soligor Zoom Slide Duplicator Type II T-2. With variable maginfication 1.0 - 2.0 X.

Box is a bit worse for wear but the lens is in excellent condition.

Seems also to have an body adaptor from 42mm screw type to 44 mm screw type.

 

Below shows how the slide fits in. The photo is from a more modern one but exactly the same front end.
