Set 28 weeks after the initial Rage Virus outbreak, the story follows the US-led NATO forces attempt to establish a safe zone in London. Don Harris (Robert Carlyle) and his wife Alice, along with other survivors, initially hide in a countryside cottage during the outbreak. Their children, Tammy and Andy, were abroad during the outbreak and later return to the fortified safe zone, District One, on the Isle of Dogs. Ignoring warnings, the siblings venture outside the perimeter, inadvertently reintroducing the virus and triggering a new wave of infection. The film depicts the chaos and horror as the military and survivors struggle against the infected, with intense action sequences and graphic depictions of the viruss effect
