EX - Booklet is in perfect condition. Digipak has no wear whatsoever.
VG+ - Booklet has wear or meaning a tab indent. Digipak has some wear.

NM and EX - CD has no scratches barely played. EX may have a light fingerprint or mark that can be removed
VG+ - CD has some small minor scratches but not affecting play.
VG - CD has a bit more scratches than VG+ but also not affecting play.
