Brass musical box - mother of pearl and brass - engraved "lorraine"

Such an interesting little musical box.   It is shaped like a lighter.   "Lorraine" engraved into the brass.   Plays "La Donna".  The mechanism feels a little loose inside the box.  The tune plays happily if a bit softly and hesitantly.   There is an 8mm wide screw into the one side, another section unclips and a tubular protrusion comes out.  I dont know what these are for?   A very small chip on the mother of pearl.  The box it is in is only half a box and has been repaired with cellotape on the corners.Please dont hesitate to ask any questions.
