Please note that for certain newer consoles' and handhelds' games whose disc is pristine or very good, I did not test to confirm as working due to time, however if you would like to see a screenshot, let me know and I will try upload one if possible.

Notes

Disc Condition

Pristine means there is not a single scuff or scratch mark. Very good means there may be some very light scuff marks. Good means it may have one or two visible scratches in addition to some scuff marks maybe, but it has been tested as working. Fair means it has definitely seen better days, but it has at least been tested and working.

Case Condition

Pristine means case is not cracked or torn. Very good means there is a sign of wear, but no major cracks. Good means it may have one or more cracks, but still functions as a case. Fair means it does not hold together very well, and will be good enough just to protect the disc.

Manual Condition

Pristine means it is as good as new. Very good means it shows some sign of wear, but not torn anywhere. Good means it still has all the pages, but may have one or more torn pages. Fair means you can still read the manual, but some pages are loose, torn or missing.
