The bluetooth control module of the PS4 CUH-1100 model has two styles - one with a sticker and other one without the sticker.
Note on this item - This has been harvested from a fully working board . It has been cut out and so is still attached to the motherboard PCB . You have to desolder it and then solder it back onto your fauly board . There is no warranty on this item as an inexperienced technician can damage the sensitive bluetooth module
