This piece is part of the infamous DIE SCHWARZE SCHANDE (translated loosely as The Black Shame) propaganda campaign circulated in post-WWI Germany to stir anti-Allied and racially-charged sentiment during the Rhineland occupation. The medal caricatures French colonial troops (specifically African soldiers) and reflects the extreme psychological warfare of the time.
